Free T4 measures the unbound portion of thyroxine circulating in your blood, the fraction actually available to enter cells. Because the thyroid gland produces mostly T4, it is the clearest single read on what your thyroid itself is putting out.

What Free T4 measures
Your thyroid produces thyroxine, or T4, and releases it into the bloodstream. Most of it immediately binds to carrier proteins and is held in reserve. Only a small unbound fraction is free to enter tissues and act on cells, and that is what this test measures.
Free T4 is used instead of total T4 because carrier protein levels shift with pregnancy, estrogen medications, and liver conditions. Total T4 moves with them and can look abnormal when thyroid function is fine. The free fraction is far less affected, so it reflects true thyroid output.
What high or low levels mean
Free T4 is almost always interpreted next to TSH, the pituitary signal that tells the thyroid how much hormone to produce. The relationship between the two is what identifies the pattern.
High TSH with low Free T4 indicates an underactive thyroid, with the pituitary pushing harder to compensate. Low TSH with high Free T4 indicates an overactive thyroid. An abnormal TSH with a normal Free T4 suggests a subclinical pattern, where the pituitary has registered a change before hormone output has visibly shifted.
Low Free T4 on its own can reflect hypothyroidism or under-replacement on thyroid medication. High Free T4 can reflect hyperthyroidism, over-replacement, or certain phases of thyroiditis.

Why measure free T4 instead of total T4?
Total T4 includes hormone bound to carrier proteins, and those protein levels change with pregnancy, estrogen medications, and liver disease. That can make total T4 look abnormal when the thyroid is working normally. The free fraction sidesteps that problem.
My TSH is abnormal but my Free T4 is normal. What does that mean?
That combination usually indicates a subclinical pattern. The pituitary has detected a shift and adjusted its signal, but hormone output has not yet moved out of range. It is typically monitored rather than treated immediately.
Can supplements affect the result?
Yes. Biotin, common in hair and nail supplements, interferes with many thyroid assays and can distort results in either direction. Most labs advise pausing biotin for two days before the draw.
